what to expect and the paperwork to bring with you

WHAT TO EXPECT

During your first rheumatology visit, Dr. Maldonado will have access to any medical records your referring provider may have sent. If there are any records, radiology, or labs you think she may not have, you are welcome to bring them with you to your visit.

At the reception desk, you will be checked in. We ask that you bring your insurance cards and ID. There may be additional paperwork. Copay is taken at time of service.

The Medical Assistant then takes you to an exam room to get a medical history from you, such as allergies, medications, and vital signs. 

Dr. Maldonado will then likely perform an exam depending on your symptoms and conditions. She will also go over your medical history and notes, medications, and answer any questions you may have.

Prescriptions, orders, and referrals will be given as needed. She'll let you know when she'd like to see you again for your follow-up visit.

FORMS

New patients will want to fill out both the New Patient Packet and Health Questionnaire to be prepared for your first visit.

The Health Questionnaire is filled out at every visit. Feel free to bring it completed to your next appointment.

If you are wishing to release records to another provider, please fill out the Records Release.

Any completed documents can be sent via our Portal, fax, or mail. If coming in person, you're welcome to bring them with you at the time of your visit.
